Thyroikd Cancer Associated with Benign Thyroid Lesion




Abstract
This report represents a clinical study of the 84 cases of thyroid cancer associated with benign thyroid lesion which had been treated surgically at the Department of Surgery, Guro Hospital, Medical College of Korea University, from January 1984
through
December 1992.
@ES The results are as follows:
@EN The age of the patients ranged from 21 to 72 years, with 77.4%occurring between 30 and 59 years, and the sex ratio between male and female was 1:9.5. On preoperative evaluation, the incidence of euthyroidism and hyperthyroidism was 91.7% and
8.3%
respectively, while the incidence of cold nodule and hot nodule was 88.9% and 7.4%. Benign lesions existed on the same lobe with malignant nodule in 60.7%, and on the contralateral lobe in 29.8%. The pathologic finding of benign lesions were
nodular
hyperplasia(58.3%), chronic thyroiditis(14.3%), Hashimoto's thyroiditis(9.5%), follicular adenoma(9.5%), Graves' disease(5.9%) and thyroglossal duct cyst(2.5%).
In the clinical characteristic study, there seemed to be no statistical difference in age, tumor size and invasiveness except in sex between thyroid cancer associated with benign lesion or not. Invasiveness of thyroid cancer associated with
Graves'
disease was significantly higher than that of Hashimoto's thyroiditis, chronic thyroiditis and follicular adenoma.
In conclusion, we should carefully examine the contralateral lobe of the benign lesion and treat vigorously the thyroid cancer which was associated with Graves' disease.
